How to Claim Your UnitedHealthcare Oxford Gym Reimbursement (2026 Guide)

If you have a UnitedHealthcare Oxford plan in New York, you are probably entitled to $200 every six months for going to the gym. That is $400 a year, for workouts you are already doing. The program is called Sweat Equity, and most eligible members never file a single claim.

The benefit is not hidden, exactly. It is just buried in plan documents nobody reads, and the filing process asks enough of you that most people give up partway through. This guide walks the whole thing end to end.

What you get

Oxford reimburses up to $200 per six-month period. There are two periods in a calendar year, so the annual maximum is $400. You claim per period, not per year, which matters more than it sounds: each period is its own claim, with its own deadline, and missing one does not roll forward into the next.

Spouses, domestic partners, and dependents aged 13 and over may also qualify if they are enrolled, at up to $100 per period.

The 50-visit threshold

You need 50 qualifying visits within the six-month period. That works out to roughly two visits a week. The visits must be to a qualifying facility, and they must fall inside the period you are claiming for.

What counts:

  • Gym and fitness facility visits
  • Fitness classes — yoga, pilates, CrossFit, spinning
  • Organized events such as marathons, 5Ks, and triathlons

What does not count:

  • Tennis clubs, country clubs, and social clubs
  • Sports teams
  • Weight loss clinics and spas
  • Equipment, clothing, and vitamins
  • Private lessons
  • Physical or rehabilitative therapy

The distinction that trips people up most often is the club category. A tennis or country club membership does not qualify even if you use the gym inside it.

The 120-day deadline

This is the number that matters

You have 120 days from the end of the period to file, per the current form. Older Oxford paperwork circulated a longer window that no longer applies. Anyone still working from the superseded number files late and gets denied.

Late filing is the single most common reason Sweat Equity claims are rejected. The paperwork can be flawless and it still will not be paid if it arrives on day 121. If you are claiming for a period that ended June 30, your window closes in late October.

The documents you need

Oxford asks for three things, and an incomplete packet is rejected as a whole rather than paid partially:

  1. A completed Sweat Equity form — one per person, per six-month period.
  2. Proof of payment, meaning a receipt or a bank withdrawal statement showing your membership dues.
  3. A gym brochure describing the cardiovascular equipment or class offerings at your facility.

That third item surprises people. Oxford wants evidence that the facility actually provides cardiovascular exercise, so a rate card or a class schedule is usually enough.

The signature requirement

A representative from your gym has to sign the form. Not you — someone at the facility, attesting to your attendance. In practice this means catching a manager at the front desk during a quiet moment and asking them to sign something they may never have seen before.

It is worth calling ahead. Larger chains often have someone who handles these regularly. Smaller studios may need you to explain what the form is.

Where to send it

The completed packet goes by mail to the Oxford Sweat Equity Program, P.O. Box 31386, Salt Lake City, UT 84131. Some members can upload through myuhc.com instead, depending on plan. Processing typically takes four to six weeks, after which a check arrives.

Why most people never finish

Count the steps: track 50 visits over six months, keep payment records, obtain a brochure, get a signature from your gym, complete a form, and mail a physical packet to Utah inside a 120-day window. Every one of those is small. Together they are enough friction that, in our survey of 124 insured professionals, 75.0% had never successfully claimed.
