- Completed Oxford Sweat Equity Reimbursement Form (NY variant)
- Visit record: computer-generated printout of dated visits, or manually completed visit chart
- Proof of payment (receipt, or automatic bank withdrawal statement)
- Facility brochure or class flier describing cardio equipment or benefits (stored once per facility, auto-attached)
- Facility representative signature (rendered into the form; single period; never reused)
